- Connect clients with appropriate services and providers.
- Assist in understanding and using funding.
- Monitor service delivery and client progress.
- Maintain accurate records and documentation.
- Advocate for client needs and preferences.
- Build relationships with service providers and stakeholders.
- Support clients in developing skills and independence.
- Manage crises or changes in client circumstances.
- Ensure services align with client goals and legal requirements.
**Job Types**: Full-time, Permanent
Pay: Up to ₹20,000.00 per month
