Provide care coordination services from QurHealth to patients (users) who subscribe via QurBook App and/or by meeting patients at a hospital location.
- Provide “First Point of Contact” for assigned patients (users) to guide them, assist them in all medical and healthcare needs within the scope of the subscriptions, enter patient data in Qurplus, health monitoring during follow-up calls.
- Should have experience in working as a “Care Team” to a large pool of patients.
- Educate Patients on Care Concierge services, Care Regimen steps to make sure patients are upto date on care adherence.
- Work with Patients to ensure patient’s profile is completed within 3 days of download.
- Should be highly detail oriented and document all conversations with patients for auditing and patient service history.
- Should maintain pleasing, empathetic and sympathetic attitude towards patients care coordination.
- Should be a self-starter and be on top of the patient care coordination duties.
- Should know to involve appropriate Care Team (Physicians, specialist doctors, Dietitian, Nutritionist, Psychiatrists, etc.,) based on health condition of patient.
- Should be available to work 6 days a week and work on Sundays/holidays in rotation basis.
- Work with Sales Team and OPS team to ensure patient information/plan activities are fully documented and shared with respective hospitals/clinics.

Pay: ₹15,000.00 - ₹25,000.00 per month
