**Job Title: Patient Care Coordinator**

**Responsibilities**:
1. _Tele-Counseling:_ Conduct tele-counseling sessions to address patient inquiries, provide information, and offer support. Ensure a compassionate and empathetic approach in all interactions with patients.

2. _Appointment Scheduling: _ Efficiently manage and organize patient appointments, ensuring optimal utilization of healthcare resources. Coordinate with healthcare providers to accommodate patient scheduling preferences.

3. _Communication: _Facilitate clear and effective communication between patients, doctors, and other healthcare professionals. Relay important information to patients regarding appointments, treatment plans, and any relevant updates.

4. _Facility Arrangements: _Arrange necessary facilities and services for patients, including transportation and accommodation when required. Collaborate with relevant departments to ensure a comfortable and supportive environment for patients.

5. _Service Coordination: _Assess individual patient needs and coordinate healthcare services accordingly. Collaborate with multidisciplinary teams to provide comprehensive care and address patient concerns.

6. _Reporting: _Provide regular reports to management on patient care activities, including challenges, successes, and areas for improvement. Contribute to the development and implementation of strategies to enhance patient care quality.

7. _Front Desk Coordination: _Collaborate with front desk staff to ensure a smooth flow of patient interactions and efficient check-in/check-out processes. Address any issues or concerns related to the front desk operations for primary care.

8. _Quality Improvement: _Actively participate in the identification of areas for improvement in patient care services. Propose and implement suggestions for the development and enhancement of quality care.

**Qualifications**:

- Master's degree in healthcare administration, Social Work, Nursing, Psychology or a related field.
- Proven experience in a similar role within a healthcare setting.
- Strong organizational and communication skills.
- Ability to multitask and work effectively in a fast-paced environment.
- Empathy and a patient-centric approach.

**Salary**: ₹18,000.00 - ₹30,000.00 per month

Schedule:

- Day shift
- Night shift

Supplemental pay types:

- Overtime pay
- Performance bonus

**Experience**:

- total work: 3 years (required)
- Psychiatry / Psychology/ Healthcare: 3 years (required)

Ability to Commute:

- Bengaluru, Karnataka (required)

Ability to Relocate:

- Bengaluru, Karnataka: Relocate before starting work (required)
