Patient Care Manager Overview: As a Patient Care Manager, you will play a crucial role in overseeing and coordinating the delivery of patient care services within the healthcare facility. You will be responsible for managing a team of healthcare professionals and ensuring that high-quality and compassionate care is provided to patients.Key Responsibilities:
  • Supervise and mentor a team of healthcare staff, including nurses and medical assistants
  • Develop and implement patient care plans in collaboration with physicians and other medical professionals
  • Ensure compliance with healthcare regulations and standards
  • Coordinate patient admissions, transfers, and discharges
  • Assess patient needs and develop appropriate care strategies
  • Conduct regular patient assessments and follow-ups
  • Oversee medication management and administration
  • Collaborate with other departments to ensure seamless patient care
  • Handle patient and family concerns and complaints
  • Participate in quality improvement initiatives

Required Qualifications:
  • Bachelor's degree in Nursing or related field
  • Valid Registered Nurse (RN) license

  • Effective communication and interpersonal abilities
