Key Responsibilities

  1. Care Coordination

· Develop individualised care plans based on assessments and clinical data.

· Coordinate all aspects of the individual's and their families healthcare, including doctor's appointments, therapy sessions, lab tests, and hospital visits.

· Monitor progress, update plans, and ensure continuity of care across services.

· Communicate with healthcare providers to ensure cohesive treatment plans.

· Avoid duplication of services or conflicting treatments.

2. Medication Management

· Keep track of prescribed medications, refills, dosages, and schedules.

· Monitor for side effects or interactions.

· Educate the individual and caregivers about medication use.

3. Health Monitoring

· Track symptoms, vitals, and overall health status daily or weekly.

· Report any changes or concerns promptly to the medical team or family.

· Support chronic disease management (e.g., diabetes, hypertension).

4. Patient Advocacy & Support

· Educate patients and families about diagnoses, treatment plans, and self-care.

· Serve as a primary contact for patient concerns and support needs.

· Empower patients to achieve health goals through motivational interviewing and counseling.

· Help navigate difficult healthcare decisions and communicate clearly with providers.

5. Wellness & Lifestyle Support

· Encourage and help maintain healthy routines: nutrition, physical activity, sleep, and stress management.

· Coordinate with nutritionists, therapists, or wellness professionals as needed.

6. Emotional & Social Support

· Build a trusting relationship with the patient to offer companionship and emotional support.

· Recognise signs of anxiety, depression, or cognitive decline and coordinate appropriate support.

7. Collaboration & Communication

· Work with physicians, nurses, social workers, and allied health professionals to deliver integrated care.

· Participate in interdisciplinary case conferences and care planning meetings.

· Provide regular updates to family members (as agreed upon).

8. Documentation & Compliance

· Organise and maintain all health records and documentation.

· Assist with insurance claims, billing questions, and navigating healthcare coverage.

· Maintain accurate, timely, and confidential documentation of all care management activities.

· Ensure compliance with organisational protocols and healthcare regulations.

9. Quality Improvement

· Track outcomes and contribute to quality improvement initiatives.

· Identify gaps in care and recommend systemic improvements.

Qualification

· Background in Medicine, nursing, Allied healthcare, healthcare management

· Bachelor's or Master's degree in Nursing, Social Work, Public Health, or related field.

· First aid/CPR certification (preferred).

Experience

· 2+ years in care coordination, case management, or a related healthcare role.

· Experience working with elderly or chronically ill patients in a personal or professional setting.

· Experience in chronic disease management, mental health, or geriatric care is a plus.

Skills
