Job Title: Healthcare Coordinator ( Nursing)

Location: Care Bridge Home, Kochi, Ernakulam, Kerala

Job Summary:

Care Bridge Home is seeking a dedicated Healthcare Coordinator ( BSc/GNM) to oversee and coordinate our home health care services. This role involves both hospital-based and community-based responsibilities, requiring strong clinical skills, leadership, and management abilities to ensure seamless healthcare services for patients.

Candidates with experience and interest in developing community networks and collaborations will have an added advantage for this position. This position is open to only who is having a Degree in BSc nursing with expereince.

Key Responsibilities:

  • Coordinate and oversee home care services in collaboration with hospital and community healthcare teams.
  • Facilitate clear and effective communication among patients, families, caregivers, and healthcare professionals.
  • Provide induction and ongoing training and support to care staff to uphold high standards of patient care.
  • Develop and implement individualized care plans based on patient assessments and needs.
  • Manage staff scheduling, allocation, and patient follow-ups to ensure seamless and efficient service delivery.
  • Monitor patient progress and proactively address medical or care-related issues as they arise.
  • Maintain accurate and up-to-date records and documentation of all patient care activities.
  • Travel as necessary to meet service delivery requirements.
  • Engage with hospitals, clinics, and other healthcare institutions to generate patient leads and referrals.
  • Establish and nurture partnerships with hospitals and healthcare providers to expand service offerings.
  • Promote Care Bridge Home's services through outreach to institutions and organizations.
  • Identify opportunities for growth and service enhancement to maximize patient care offerings.
  • Maintain consistent follow-up with potential clients and partners to foster relationships and sustain communication.
  • Arrange meetings and presentations with prospective clients and stakeholders to discuss services and partnership opportunities.

Requirements:

  • Education: BSc/GNM Nursing
  • Experience: Minimum of 1 year in a healthcare setting
  • Strong leadership, management, and problem-solving skills.
  • Excellent communication skills in Malayalam and English.
  • Ability to multitask and work in a dynamic healthcare environment.
  • Willingness to travel and handle both hospital and community-based responsibilities.
  • Must be locally available in Kochi,Ernakulam Kerala.

Employment Type:
