Job Description**Roles & Responsibilities**

**Pre conversion**

**1**

To be responsible for IP patient's counselling for present day's discharge**2**

To take ICU rounds post discharge counselling and filter potential patients**3**

To meet doctors basis their generated leads and take their consent for required homecare service for patient**4**

To do assessment in portal/ or Manual of every converted lead. Assessment form to be duly filled**5**

Coordination with other spokes in case lead has to be transferred in other spokes ( after conversion )**6**

Making visits for assessment for out of hospital leads and end to end coordination with OOH patients till lead's closure.**7**

Coordinating with Nobin for CCN packages, discounts on CCN lead and offer the final price to patient till lead closure.**8**

All three documents to be duly filled and signed - Registration, KYC & Patient Agreement (Only for CCN) + advance after conversion**Post conversion**

Handover the relevant documents (Registration, KYC & Patient Agreement) & forms to UC**2**

Creating service request after conversion and forward the lead to UC and service operations to be taken up by UC further.**3**

If ATL get escalation in any form by running patients, ATL will post the escalation in respective spoke's/ service group and operations team will take up the escalation and give solution.**BAU**

To maintain patient wise ICU tracker for their respective units and share with stakeholders on daily basis.**2**

To keep the doctor updated on running patients.**3**

To align ICU TLs/ Nursing staffs and other stakeholders.**4**

To be responsible for given individual targets and work with team on MOM growth plans**5**

To provide required data to ASM/AH for weekly/daily/monthly calls.**6**

To become the part of all trainings/ meetings or calls are planned for sales/ATL team.**7**

Information of service dis-continuation to be timely mentioned in the Service Stopped group
