**Job Summary**:
The Patient Care Coordinator is responsible for ensuring a seamless patient experience by managing all aspects of patient care coordination. This role involves direct interaction with patients, families, and healthcare providers to facilitate the delivery of high-quality healthcare services. The Patient Care Coordinator will play a pivotal role in patient advocacy, appointment scheduling, converting leads to deals, and the coordination of care plans to enhance patient outcomes and satisfaction.

**Key Responsibilities**:

- **Patient Interaction**:

- Serve as the primary point of contact for patients and their families, providing compassionate support and guidance.
- Address patient inquiries, concerns, and complaints promptly and professionally.
- Educate patients and their families about healthcare procedures, treatment plans, and medication instructions.
- **Care Coordination**:

- Coordinate patient appointments, follow-up visits, and referrals to specialists.
- Collaborate with healthcare providers to ensure a comprehensive care plan is in place for each patient.
- Monitor patient progress and ensure continuity of care by maintaining up-to-date medical records and documentation.
- **Administrative Duties**:

- Manage patient files and ensure all documentation is accurate and up-to-date.
- Handle patient intake, including gathering medical history and insurance information.
- Assist with billing and insurance claims to ensure timely processing and resolution of any issues.
- **Lead Management**:

- Receive and manage inbound calls from prospective patients, answering queries and providing information about services.
- Identify potential leads and engage in converting them into actual patients through effective communication and follow-up.
- Track and report on lead conversion metrics to improve strategies and processes.
- **Team Collaboration**:

- Work closely with medical, nursing, and administrative staff to ensure efficient workflow and patient care processes.
- Participate in team meetings and contribute to the development of patient care strategies.
- **Quality Improvement**:

- Identify opportunities for improving patient care and satisfaction and implement appropriate changes.
- Stay informed about the latest healthcare practices and protocols to ensure compliance with industry standards.

**Qualifications**:

- **Education**:

- Bachelor’s degree in Healthcare Administration, Nursing, or a related field preferred.
- Relevant certification (e.g., Certified Patient Care Coordinator) is a plus.
- **Experience**:

- Minimum of 2 years of experience in a healthcare setting, preferably in patient care coordination or a related role.
- Experience with electronic medical records (EMR) systems.
- **Skills**:

- Excellent communication and interpersonal skills.
- Strong organizational and time-management abilities.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook).
- Ability to work independently and as part of a team.
- Detail-oriented with a high degree of accuracy.
- Proven track record in managing leads and converting them into deals.

Pay: ₹10,752.38 - ₹25,000.00 per month
