The Patient Care Coordinator Nurse (Junior Level) is a full-time position based in Mumbai, responsible for facilitating seamless patient care and communication between multidisciplinary teams and patients. Qualifications and Skills

- Expertise in care coordination to streamline patient referrals, transitions of care, and documentation while reducing fragmentation in care delivery.
- Ability to provide clear, accurate, and compassionate patient education to promote understanding of treatment plans and informed decision-making.
- Competence in patient counseling, offering emotional support to patients and families throughout the continuum of care and aiding them in coping with diagnoses or procedures.
- Talent for time management, prioritizing multiple clinical tasks, and maintaining accurate electronic health records and care plans for efficient workflow.
Roles and Responsibilities
- Serve as the primary nursing contact for patients, addressing clinical queries, care needs, and coordinating appointments or follow-up services.
- Collaborate with multidisciplinary medical teams to implement and monitor individualized care plans tailored to patient needs across specialties.
- Educate patients and their families regarding diagnoses, treatments, medications, and post-discharge instructions to ensure superior health outcomes.
- Support patients during admission, transfer, and discharge processes, coordinating with relevant departments to ensure smooth transitions.
- Prioritize patient safety initiatives by adhering strictly to infection control protocols, documentation guidelines, and institutional policies.
- Conduct patient counseling sessions to provide reassurance, address concerns, and facilitate stress reduction during hospitalization.
- Monitor patient progress, promptly report changes in condition to the appropriate medical team, and advocate for their needs.
- Maintain meticulous patient records and facilitate seamless flow of information among healthcare professionals for coordinated and timely interventions.
