About the Role
As a Service Coordinator - Program Support Specialist, you will be responsible for coordinating programs and services to eligible individuals on a group basis. This will involve working closely with community agencies, network service providers, and identifying new services available to eligible individuals. You will also be responsible for monitoring the ongoing provision of services, managing the provision of supportive services, and developing and maintaining a Resource Directory.
Key Responsibilities
- Program Coordination
- Coordinate programs and services to eligible individuals on a group basis.
- Work closely with community agencies and network service providers.
- Identify new services available to eligible individuals.
- Monitor the ongoing provision of services.
- Manage the provision of supportive services.
- Develop and maintain a Resource Directory.
Individual Coordination
- Coordinate services for participants on an individual basis.
- Conduct person-centered assessments and screenings.
- Provide quality customer service to all eligible individuals.
- Assist participants in acquiring and utilizing desired community services.
- Follow up and monitor participant progress.
Documentation and Reporting
- Complete timely electronic and written documentation.
- Submit documentation needed for funder and quality Performance Reports.
Requirements
To be successful in this role, you will need:
- A high school diploma or GED.
- Two or more years of experience in a social service delivery with elderly, people with disabilities, and/or family population.
- Demonstrated working knowledge of supportive services and other resources in the area served by the project.
- Demonstrated ability to advocate, organize, problem-solve, and provide results for the participants served.
- Good communication, comprehension, and interpersonal skills.
- A working knowledge of Windows, Microsoft Office Suite, and the World Wide Web.
