Role Summary
We are seeking a skilled and compassionate Patient Support Coordinator to join our team. In this critical role, you will provide vital support to patients and their families as they navigate challenging situations.
Key Responsibilities
- Provide emotional and psychosocial support to patients and families coping with illness, disability, or trauma.
- Help patients adjust to hospitalization, diagnosis, or chronic health conditions by assessing their social, emotional, and economic needs.
- Develop care plans and coordinate follow-up services to ensure seamless continuity of care.
- Connect patients with external agencies for financial aid, rehabilitation, shelters, home care, or government schemes.
- Collaborate with NGOs and social service organizations to provide comprehensive support.
Requirements
- Excellent communication and counseling skills to effectively engage with patients and families.
- Empathy, patience, and emotional intelligence to provide sensitive support during challenging times.
- Strong documentation and reporting ability to maintain accurate records and share information with clinical teams.
- Ability to work effectively in multidisciplinary teams and contribute to collaborative problem-solving.
