Job Description

As a patient advocate, you will play a vital role in providing emotional and psychosocial support to patients and families coping with illness, disability, or trauma. You will help patients adjust to hospitalization, diagnosis, or chronic health conditions by assessing their social, emotional, and economic needs. Your expertise in case management, counseling, and crisis intervention will enable you to develop care plans and coordinate follow-up services that cater to the unique needs of each patient.
