Job Summary:

We are seeking a skilled Rehabilitation Specialist to join our team. In this role, you will work closely with patients to develop and implement personalized rehabilitation plans that promote optimal recovery and independence.

Responsibilities:

  • Conduct comprehensive assessments of patients' physical, cognitive, and emotional needs
  • Develop and implement individualized rehabilitation plans that address patient goals and objectives
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Provide education and support to patients and their families on healthy habits, injury prevention, and disease management
  • Stay up-to-date on best practices and research in rehabilitation therapy through ongoing education and training
  • Maintain accurate and detailed records of patient progress and treatment plans
  • Communicate effectively with healthcare professionals, patients, and family members to ensure high-quality care

Requirements:

  • Bachelor's degree in Occupational Therapy or related field
  • Current licensure as an Occupational Therapist (OT) in good standing
  • Proven experience in rehabilitation settings, preferably with at-risk populations
  • Strong analytical and problem-solving skills, with ability to think critically and creatively
  • Excellent communication and interpersonal skills, with ability to build rapport with diverse patient groups
  • Ability to work independently and collaboratively within a fast-paced team environment
