
Healthcare Coordinator Specialist

We are seeking an experienced and compassionate Healthcare Coordinator Specialist to join our team. In this role, you will provide high-quality patient care and support services to individuals coping with illness, disability, or trauma.
Key Responsibilities:- Patient Support: Offer emotional and psychosocial support to patients and their families in need of comfort, guidance, and reassurance.
- Case Management: Assess patient needs, develop personalized care plans, and coordinate follow-up services to ensure a seamless transition from hospitalization to community-based care.
- Counseling & Crisis Intervention: Provide individual and family counseling during times of grief, stress, or crisis, offering a safe space for patients to express their emotions and concerns.
- Referral Services: Connect patients with external agencies providing financial aid, rehabilitation, shelter, home care, or government schemes, promoting access to essential resources and support.
- Discharge Planning & Continuity of Care: Collaborate with healthcare professionals to ensure smooth discharge planning, arranging support systems at home or in the community, and coordinating continuity of care.
- Communication Skills: Possess excellent communication and counseling skills to effectively interact with patients, families, and healthcare teams.
- Empathy & Emotional Intelligence: Demonstrate empathy, patience, and emotional intelligence when working with vulnerable populations.
- Community Coordination Skills: Showcase casework and community coordination skills to connect patients with necessary resources and support.
- Knowledge of Government Schemes: Have knowledge of government health and welfare schemes to inform patients about available resources.
- Documentation & Reporting: Maintain accurate patient case notes and service records, preparing social assessment reports for clinical teams.
- Ongoing Training & Development: Participate in regular training sessions to enhance your skills and knowledge in patient care and support services.
- Mentorship Opportunities: Receive mentorship and guidance from experienced colleagues to help you grow professionally.
