We are a specialized subcontractor company dedicated to delivering high-quality rehabilitation services—including Physical Therapy, Occupational Therapy, and Speech Therapy—to homecare agencies. Our mission is to support patient recovery and independence by coordinating timely, effective, and compliant in-home rehab services. As a Remote Care Coordinator, you will be a key liaison between patients, therapists, and partnering homecare agencies.
Position Summary
The Remote Case Manager plays a critical role in ensuring seamless communication, scheduling, and case management between our therapists, homecare agencies, and patients. This role involves overseeing the referral intake process, assigning cases to field therapists, tracking visit completion, verifying documentation, and maintaining regulatory and contractual compliance.
This position requires exceptional organizational skills, strong communication, and a deep understanding of home health operations or rehab services.
Key Responsibilities
Manage incoming referrals from partner homecare agencies and process them in a timely manner
Assign cases to therapists based on location, specialty, and availability
Coordinate and schedule start-of-care visits, evaluations, and follow-up visits
Communicate effectively with therapists, agency staff, and patients to ensure timely service delivery
Monitor therapist caseloads and ensure visit compliance per plan of care and insurance requirements
Track documentation submission and ensure timely upload to EMR systems
Verify therapist notes and documentation for quality and accuracy; escalate issues as needed
Serve as the point of contact for case-related inquiries from agency partners
Support therapists with logistics, documentation follow-up, and case troubleshooting

Required Qualifications
Minimum of 2 years' experience in care coordination, scheduling, or case management (home health or rehab preferred)
