**Role Overview**: We are seeking a motivated and enthusiastic Intern to support our Case Management team. As a Case Management Intern, you will have the chance to work closely with experienced professionals, collaborate with healthcare providers, and contribute to the effective management of patient cases. This internship offers a unique opportunity to develop essential skills in healthcare coordination, communication, and problem-solving.

**Key Responsibilities**:
Assist in managing a portfolio of cases under the guidance of experienced Case Managers.

Participate in proactively coordinating with healthcare providers to ensure seamless care delivery to patients.

Learn and adhere to relevant healthcare regulations, best practices, and internal policies and procedures.

Accurately update and maintain case records and documentation. Collaborate with team members to ensure accurate and up-to-date case information is readily available.

Work closely with clients to understand their needs and contribute to the creation of treatment plans.

Support the coordination of discharge plans in collaboration with service providers.

Participate in interdisciplinary team meetings to enhance communication and case coordination.

Minimum Qualifications:
Currently pursuing a Bachelor's degree in Nursing, Social Work, Healthcare Administration, or a related field.

Genuine interest in healthcare case management and patient-centered care. Basic understanding of healthcare regulations and protocols.

Strong attention to detail and organizational skills.

Effective communication skills, both written and verbal.

Willingness to learn and work collaboratively in a fast-paced environment.

Preferred Qualifications:
Enrollment in a Master's degree program in a relevant healthcare field (optional). Knowledge of electronic medical records (EMR) systems.

Demonstrated interpersonal skills and a compassionate approach to patient care.

Familiarity with telehealth platforms and digital healthcare tools. Strong problem-solving abilities and critical thinking skills.
