Position: Staff Nurse Home Care
Function: Clinical Services
Reports To: Ward Manager
1. Job Purpose
- Responsible to deliver a professional, safe and quality patient care through the nursing process of assessment, planning, implementation and evaluation
- Responsible for effective communication with patients, relatives and members of the health care team
- Provide medical care and treatment to patients in their home
- Meet with patients shortly after they are discharged from the hospital
- Evaluate compliance to accreditation standards and quality in general and ensure implementation of corrective and preventive action in patient care areas including infection control practices throughout the facility
- Responsible for patient and family satisfaction
- Adhere to all clinical and nursing protocol as per Hospital Policies of Sir H N Reliance Foundation
2. Accountabilities
Key Responsibilities: Service
- Provide care in co-ordination with the physician and other health care workers
- Assess the patients condition , health progress and needs
- Manage patient as per scope of services
- Identify care issues and makes recommendations about changes to the patient care plan as per consultant recommendation
- Communicate with family care givers, Listen to their concerns and update them on the patient condition.
- Keep records for future reference
- Maintain the Documentation of care provided accurately in EMR
- Ensure that patient privacy and confidentiality is duly protected at all the times
- Treat patients and their family with courtesy, respect, and caring behavior
- Take immediate action to resolve any issues, complaints or distress expressed in any way by the patient or their family with in her limits and escalate to higher level
- Assess the patient for risk of developing pressure ulcer and implement pressure ulcer preventive measures
- Administer medication to the patient as per the doctors prescription and document the same in EMR
- Ensure all clinical consents are uploaded in patient EMR
- Work as a liaison between the patient and treating physician if the patient is registered to RFH
- Escalate the patients condition to treating physician if any.
- To communicate the observations, non- compliance, issues and challenges compiled from all possible information sources towards compliance to quality standards
- Compile and summarize data for presentation at monthly departmental meetings and quarterly meetings as requested.
- Work closely with nursing manager, Head of the department and doctors to ensure a high standard of nursing care
- Generate regular reports regarding the adherence to quality standards, achievements and possibility of improvement, and forwards copy of line manager.
- Promote use of evidence based practice
- Ensure in readiness of home care Kit at all times.
- Ensure the compliance in completing the process of home are services.
- Perform any other duties as directed by the line manager
- Any other responsibility as assigned by the ward manager
Key Responsibilities: Growth
- Ensure patient satisfaction is as per the parameters set by the hospital from time to time
- Engage in aligning with the processes, policy, vision & mission of the hospital
- Enhance Patient Care and Patient Relatives experience to make RFH the preferred choice
Key Responsibilities: Quality
- Ensure Nursing and infection control and other quality indicators are within the defined bench mark
- Actively participate and take accountability for implementation of all quality programs to ensure continuous quality improvement
- Actively participate in infection control and prevention activities, campaigns, awareness and sessions
- Ensure nursing clinical quality standard and practice are compiled, maintained and evaluate when necessary.
- Reporting of deficiencies or scope of improvement.
Key Responsibilities : Finance
- Ensure optimal utilization of available resources (drugs and consumable)
- Ensure proper utilization of ward stock items and maintain its appropriate charging or stock consumption record
- Ensure safe handling of CSSD and biomedical equipments and prevent breakages
- To provide inputs clinical Services through - Quality of care
- Monitor and ensure optimal utilization of available resources
Key Responsibilities: People
- Initiate & coordinates safety practices
- Attend all in service Nursing/hospital trainings
- Assist in orientation of new nursing staff and supervise them for safe patient care
- Supervise PCAs / PTAs and ensure patient care
- Ensure effective patient care delivery and achievement of desired patient outcomes in
- collaboration with other professional disciplines
- Engage in training and educational program for freshers and new joinees to impart the protocols and hospital policies
- Ensure effective communication with the health care team members by ensuring

- Read back and verify Verbal /Telephonic orders/ critical alert reporting. No verbal order to be followed
Key Responsibilities: Innovations and Research
- Identify and suggest new ideas/ feedback to enhance departmental operations and patient care
- Plan and participate in clinical education, research, test medication and clinical tests to advance the practice of nursing as well as academic and teaching activities as required by the hospital.

EDUCATIONAL QUALIFICATIONS:
Necessary: B.Sc. Degree Should have nursing council registration
RELEVANT EXPERIENCE:
Necessary: 1-3 years of nursing clinical experience
