Position : Staff Nurse ICUReports To : Ward Manager 1. Job PurposeResponsible to deliver a professional, safe and quality patient care through the nursing process of assessment, planning, implementation and evaluation.Responsible for effective communication with patients, relatives and members of the health care teamResponsible for patient and family satisfactionAdhere to all clinical and nursing protocol as per Hospital Policies of Sir H N Reliance Foundation2. AccountabilitiesKey Responsibilities: ServiceDeliver safe and quality nursing care in conformance with the Sir HN RFH policies and processes.Ensure patient safety at all times according to hospital policiesEnsure that patient privacy and confidentiality is duly protected at all timesTreat patients and their family with courtesy, respect, and caring behaviorTake immediate action to resolve any issues, complaints or distress expressed in any way by the patient or their family with in her limits and escalate to higher levelReceive the patient, check the identification, 0rient to the admitted unit and make the patient comfortableConduct patients initial nursing assessment within 30 mints of arrival in ward and complete the assessment within 2 hours. An ongoing nursing assessment to be done in the beginning of each shift. Reassessment to be done for long stay patients after every 30 daysDemonstrate clinical skills and knowledge in the delivery of patient care on various nursing proceduresEnsure smooth functioning of the ICUComplete the pain assessment and reassessment and intervene as requiredIdentify the vulnerable patients, plan and provide the care accordinglyAssess the patient for risk of developing pressure ulcer and implement pressure ulcer preventive measuresAdminister medication to the patient as per the doctors prescription and document the same in EMRAccompany the Consultants in the clinical rounds and follow the orders correctly. Assist the doctors in bedside proceduresProvide Basic Life Support in all emergency situationsDemonstrate critical thinking skills which is essential to make accurate decisions in patient careEnsure safe care and handling of patients on invasive lines and tubesAll critical alerts to be read, verified and documented with clinical interventionCoordinate admission, transfer and discharge process ensuring adherence to defined timelinesEnsure provision of end of life care to patients those are in end stageMaintain accurate, comprehensive and timely medical recordsActivate CODE Blue if patient is unresponsiveIdentify early warning signs of the patient and activate MET calls as per the defined roleParticipate actively in case of any internal / external disaster situationsConduct hourly/frequent round as per the needs of the all patients Maintain Patient hygiene. Provide sponge bath to post-operative patients, critically ill and bed bound patientsAccompany patients with High acuity, patients with tubes and lines and bed bound patients for transferEducate and involve the patient and family regarding the plan of nursing care during the hospital stay and at the time of dischargeEnsure all clinical consents are uploaded in patient EMREnsure safe handling and handing over of all diagnostic reports at the time of discharge and document the same in discharge checklistEnsure MLC is initiated for all MLC cases and the process of discharge intimation is given to PE. All the documentation is done and followed as per hospital policy.Check MRD file updated and complete and handover the same to MRD within 24 hrs. of dischargeProvide death care to the deceased patients respectfully and handover to relatives/mortuary as required and document the sameAny other responsibility as assigned by the ICU managerKey Responsibilities: GrowthEnsure patient satisfaction is as per the parameters set by the hospital from time to timeEngage in aligning with the processes, policy, vision & mission of the hospitalEnhance Patient Care and Patient Relatives experience to make RFH the preferred choice Key Responsibilities: QualityEnsure Nursing and infection control quality indicators are within the defined bench markActively participate in infection control and prevention activities, campaigns, awareness and sessionsActively participate and be accountable for implementation of all quality programs to ensure continuous quality improvementKey Responsibilities : FinanceEffective optimal utilization of available resources ( drugs and consumable )Ensure proper utilization of ward stock items and maintain its appropriate charging or stock consumption recordEnsure safe handling of CSSD and biomedical equipments and prevent breakagesKey Responsibilities : PeopleAttend all in service Nursing/hospital trainingsAssist in orientation of new nursing staff and supervise them for safe patient careSupervise PCAs / PTAs and ensure patient careEnsure effective patient care delivery and achievement of desired patient outcomes incollaboration with other professional disciplinesEngage in training and educational program for freshers and new joinees to impart the protocols and hospital policyEnsure effective communication with the health care team members by ensuring:Hands off Shift hands off Interdepartmental transfer Between the levels of careRead back and verify Verbal /Telephonic orders/ critical alert reporting. No verbal order to be followedKey Responsibilities: Innovations and ResearchIdentify and suggest new ideas/ feedback to enhance departmental operations and patient carePlan and participate in clinical education, research, test medication and clinical tests to advance the practice of nursing as well as academic and teaching activities as required by the hospital.3.
