Position: Staff Nurse Home CareFunction: Clinical ServicesReports To: Ward Manager1. Job PurposeResponsible to deliver a professional, safe and quality patient care through the nursing process of assessment, planning, implementation and evaluationResponsible for effective communication with patients, relatives and members of the health care teamProvide medical care and treatment to patients in their homeMeet with patients shortly after they are discharged from the hospitalEvaluate compliance to accreditation standards and quality in general and ensure implementation of corrective and preventive action in patient care areas including infection control practices throughout the facilityResponsible for patient and family satisfactionAdhere to all clinical and nursing protocol as per Hospital Policies of Sir H N Reliance Foundation2. AccountabilitiesKey Responsibilities: ServiceProvide care in co-ordination with the physician and other health care workersAssess the patients condition , health progress and needs Manage patient as per scope of servicesIdentify care issues and makes recommendations about changes to the patient care plan as per consultant recommendation Communicate with family care givers, Listen to their concerns and update them on the patient condition.Keep records for future reference Maintain the Documentation of care provided accurately in EMREnsure that patient privacy and confidentiality is duly protected at all the timesTreat patients and their family with courtesy, respect, and caring behaviorTake immediate action to resolve any issues, complaints or distress expressed in any way by the patient or their family with in her limits and escalate to higher levelAssess the patient for risk of developing pressure ulcer and implement pressure ulcer preventive measuresAdminister medication to the patient as per the doctors prescription and document the same in EMREnsure all clinical consents are uploaded in patient EMRWork as a liaison between the patient and treating physician if the patient is registered to RFHEscalate the patients condition to treating physician if any.To communicate the observations, non- compliance, issues and challenges compiled from all possible information sources towards compliance to quality standards Compile and summarize data for presentation at monthly departmental meetings and quarterly meetings as requested.Work closely with nursing manager, Head of the department and doctors to ensure a high standard of nursing care Generate regular reports regarding the adherence to quality standards, achievements and possibility of improvement, and forwards copy of line manager.Promote use of evidence based practiceEnsure in readiness of home care Kit at all times. Ensure the compliance in completing the process of home are services.Perform any other duties as directed by the line managerAny other responsibility as assigned by the ward managerKey Responsibilities: Growth Ensure patient satisfaction is as per the parameters set by the hospital from time to timeEngage in aligning with the processes, policy, vision & mission of the hospitalEnhance Patient Care and Patient Relatives experience to make RFH the preferred choice Key Responsibilities: QualityEnsure Nursing and infection control and other quality indicators are within the defined bench markActively participate and take accountability for implementation of all quality programs to ensure continuous quality improvementActively participate in infection control and prevention activities, campaigns, awareness and sessionsEnsure nursing clinical quality standard and practice are compiled, maintained and evaluate when necessary.Reporting of deficiencies or scope of improvement.Key Responsibilities : FinanceEnsure optimal utilization of available resources (drugs and consumable)Ensure proper utilization of ward stock items and maintain its appropriate charging or stock consumption recordEnsure safe handling of CSSD and biomedical equipments and prevent breakagesTo provide inputs clinical Services through - Quality of careMonitor and ensure optimal utilization of available resources Key Responsibilities: PeopleInitiate & coordinates safety practices Attend all in service Nursing/hospital trainingsAssist in orientation of new nursing staff and supervise them for safe patient careSupervise PCAs / PTAs and ensure patient careEnsure effective patient care delivery and achievement of desired patient outcomes incollaboration with other professional disciplinesEngage in training and educational program for freshers and new joinees to impart the protocols and hospital policiesEnsure effective communication with the health care team members by ensuringHands off Shift hands off Interdepartmental transfer Between the levels of careRead back and verify Verbal /Telephonic orders/ critical alert reporting. No verbal order to be followed Key Responsibilities: Innovations and ResearchIdentify and suggest new ideas/ feedback to enhance departmental operations and patient carePlan and participate in clinical education, research, test medication and clinical tests to advance the practice of nursing as well as academic and teaching activities as required by the hospital.3.
