Job Summary:
We are seeking a compassionate, organized, and experienced Patient Care Manager to oversee the delivery of high-quality patient care within our facility. In this role, you will manage a multidisciplinary team, coordinate patient services, ensure compliance with healthcare regulations, and work to improve patient satisfaction and clinical outcomes.
Key Responsibilities:
Care Coordination & Oversight
- Supervise and coordinate all aspects of patient care services, ensuring that care plans meet medical standards and patient needs.
- Collaborate with physicians, nurses, therapists, and administrative staff to develop individualized care plans.
- Monitor and evaluate patient care services to ensure the highest standards of quality and safety.
Team Leadership
- Manage and support a team of nurses, care coordinators, and other healthcare professionals.
- Schedule staff, delegate responsibilities, and provide ongoing training and mentorship.
- Conduct performance evaluations and implement staff development plans.
Patient Advocacy & Experience
- Serve as a key point of contact for patients and families, ensuring their concerns are addressed promptly and compassionately.
- Promote a patient-centered culture by advocating for the needs and preferences of patients.
- Respond to patient feedback and implement improvements to enhance satisfaction.
Compliance & Documentation
- Ensure adherence to healthcare regulations, accreditation standards, and internal policies (e.g., HIPAA, JCAHO, CMS guidelines).
- Maintain accurate records, care plans, and documentation in electronic health records (EHR) systems.
- Participate in audits and quality assurance initiatives.
Process Improvement
- Analyze workflows, identify gaps in care delivery, and implement best practices to improve operational efficiency.
- Monitor key performance indicators (KPIs), such as patient satisfaction, readmission rates, and staff utilization.
- Lead or participate in quality improvement projects.
Preferred Qualifications:
- Experience in [hospital, home health, long-term care, or specialty clinic] setting
- Certification in case management (e.g., CCM or ACM) or healthcare quality (e.g., CPHQ)
- BLS/ACLS certification
- Familiarity with value-based care models and patient engagement strategies.
