Role (what the Health Coach is)
- Primary point of contact who bridges Health Professional such as doctor, dietitian, trainer (and psychologist when needed), turns their recommendations into practical daily routines, monitors adherence, and removes barriers.
Core responsibilities
- Daily readiness & coordination: Review client monitoring sheet, check CRM schedules, ensure reports/charts are current, and sync with Front Desk for walk-ins/onboarding.
- Pre-onboarding prep: Receive client details, review pre-screening forms (medical/fitness/diet/psych), sketch an initial roadmap, and pre-book doctor/dietitian/trainer slots where possible.
- Onboarding: Build rapport and set expectations; collect forms/metrics and contacts; discuss goals/challenges and flag red-flags; map the initial sequence—Doctor → Fitness → Diet → Psychology (if needed)—and lock schedules in the CRM.
- Translate plans into action: Convert specialist recommendations into simple week-by-week routines clients can follow (workouts, meals, recovery habits).
- Documentation & tools: Collect all consult summaries the same day, upload to the client drive, and share progress charts; maintain CRM notes and standard templates.
- Behaviour & habit coaching: Discuss stress, sleep, work-life balance, and recovery; identify adherence barriers and suggest workable strategies.
- Follow-up & monitoring: Weekly check-ins (calls/messages), track diet logs and training attendance/progress; run milestone reviews—Day-10 diet check, Week-3 diet review, Week-4 fitness reassessment, Month-end doctor consult.
- Team rituals: Participate in daily whiteboard updates to coordinate cross-functional changes and keep plans current.
- MDT (Multi-Disciplinary Team) escalation: When a client request is unsafe/unscientific or needs multi-disciplinary input, raise an MDT—submit the form, schedule the meeting, document outcomes, and follow up.
- Escalation paths: Medical → Doctor; diet issues → Dietitian; performance/injury → Fitness Trainer; motivation/mental health → Psychologist; ops issues per centre protocol.
- Communication setup: Establish channels and expectations (WhatsApp group, calls, CRM updates) from Day-0.
