We are seeking a compassionate and organized Care Coordinator to join our team. The Care Coordinator will play a crucial role in ensuring the seamless delivery of care services to our elderly members. This position requires strong attention to detail, excellent communication skills, and a genuine desire to make a difference in the lives of seniors and their families.
**Responsibilities**:
- Coordinate and schedule care services for elderly members, including personal care assistance, medication management and other support.
- Conduct initial assessments to evaluate needs and develop personalized care plans in collaboration with members, families, and healthcare professionals.
- Monitor clients' progress and adjust care plans as needed to ensure optimal outcomes and client satisfaction.
- Serve as the primary point of contact for members and their families, addressing inquiries, concerns, and requests in a timely and compassionate manner.
- Collaborate with doctors, caregivers, and other resources to coordinate additional services and support for clients as needed.
- Maintain accurate and up-to-date client records, documentation, and reports according to regulatory requirements and organizational policies.
- Participate in staff meetings, training, and professional development activities to enhance knowledge and skills related to care coordination.
Qualifications:
- Bachelor's degree or a related field preferred.
- Strong interpersonal skills and the ability to communicate effectively with elderly members, families, caregivers, and healthcare professionals.
- Excellent organizational skills and the ability to manage multiple tasks and priorities in a fast-paced environment.
- Compassionate, patient, and empathetic attitude towards the needs and concerns of seniors and their families.
- Proficiency in computer skills, including Microsoft Office Suite
