Life Circle is seeking a compassionate and results-driven Patient Care Advisor to join our team. In this role, you will guide patients and families through the process of accessing our home healthcare services, ensuring the best care solutions and driving sales growth. You'll be the bridge between clients and our operations team, focused on improving patient outcomes while meeting sales targets.

**Key Responsibilities**:

- Engage with patients and families to understand their needs and recommend suitable home care services.
- Generate and manage leads through various channels, following up promptly to convert opportunities.
- Coordinate assessments, caregiver interviews, and placements.
- Build strong relationships with patients and families, addressing concerns and ensuring satisfaction.
- Meet sales targets by promoting and explaining our services to potential clients.

**Skills & Qualifications**:

- Excellent communication, active listening, and empathy skills.
- Ability to manage multiple tasks with attention to detail and organization.
- Professional demeanor in sensitive healthcare conversations.
- Experience in sales and lead management is preferred.
- Strong team player with a proactive approach to problem-solving.
