**Job Summary**:
The Patient Care Coordinator serves as a liaison between patients, healthcare providers, and administrative staff to ensure seamless, high-quality care delivery. This role is responsible for managing patient communications, scheduling appointments, coordinating care plans, assisting with insurance and billing inquiries, and supporting patients throughout their healthcare journey.

**Key Responsibilities**:

- Serve as the primary point of contact for patients throughout their care experience
- Schedule and confirm appointments, tests, and follow-ups
- Coordinate referrals and prior authorizations as required
- Communicate effectively with clinical staff to ensure care plans are understood and executed
- Provide patient education regarding procedures, treatments, and available services
- Assist patients with insurance verifications, billing questions, and financial counseling
- Maintain and update patient records accurately in the electronic health record (EHR) system
- Monitor patient progress and follow up to ensure timely services and satisfaction
- Address patient concerns and escalate unresolved issues appropriately
- Promote a patient-centered, empathetic approach to healthcare delivery

**Qualifications**:

- High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare administration or related field preferred
- 1-3 years of experience in a healthcare setting, preferably in care coordination or patient services
- Familiarity with EHR systems (e.g., Epic, Cerner) and basic medical terminology
- Strong communication, interpersonal, and problem-solving skills
- Ability to multitask in a fast-paced environment and maintain attention to detail
- Knowledge of insurance processes, HIPAA compliance, and patient confidentiality practices

Pay: ₹23,000.00 - ₹35,000.00 per year
