**Position Summary**:
**Key Responsibilities**:

- **Health Assessments & Medical Care**:

- Conduct regular health assessments and physical exams for homeless individuals in the field.
- Provide on-site medical care, including first aid, wound care, and chronic disease management.
- Administer prescribed medications and manage health conditions under nurse protocol.
- **Crisis Care & Emergency Response**:

- Respond to immediate health crises, including injuries, infections, or medical emergencies.
- Provide urgent medical care and stabilize individuals for transport if necessary.
- Ensure follow-up care and referrals for emergency or urgent health issues.
- **Hygiene & Preventative Health**:

- Educate individuals about basic hygiene practices and health prevention (e.g., vaccination, hygiene, skin care).
- Provide hygiene kits and ensure access to clean water, sanitation, and medical supplies.
- **Health Referrals & Coordination**:

- Connect individuals to necessary health services, including shelters, clinics, mental health, and substance abuse treatment.
- Assist with transportation and appointments for medical care, including hospitalization if required.
- Coordinate with healthcare providers, social workers, and community organizations for holistic care.
- **Documentation & Case Management**:

- Maintain accurate medical records of assessments, treatments, and referrals.
- Track patient progress and health outcomes through follow-up visits or phone calls.
- Document interactions and ensure confidentiality in line with HIPAA and organizational policies.
- **Education & Health Promotion**:

- Provide health education on topics such as nutrition, mental health, substance abuse, and disease prevention.
- Support individuals in managing their health conditions through lifestyle and treatment recommendations.
- **Team Collaboration & Support**:

- Work collaboratively with outreach workers, social workers, and other healthcare professionals to ensure comprehensive care.
- Participate in team meetings and training sessions to enhance skills and knowledge.
