Medical Documenter

A dynamic team seeks a Medical Documenter with exceptional administrative skills to join their ranks. As a Medical Documenter, you will play a vital role in ensuring the accuracy and completeness of patient records.

  • You will review physician dictation and transcribe clinical notes into electronic medical records (EMRs) and billing systems.
  • You will prepare and assemble medical record documentation, including charts for physicians.
  • You will ensure medical record compliance through self-documentation attestation.
  • You will update patient history, physical exam, and other pertinent health information in the patient chart.
  • You will prepare and send all documentation to physicians for review and approval via authentication of detailed data entry and facility-specific procedures.
  • You will monitor lab results and screening procedures.
  • You will comply with hospital and medical facility policies, including those related to HIPAA and Joint Commission.
  • You will perform other clerical duties and tasks to improve provider productivity and clinic workflow as assigned.
  • You will assign codes to diagnoses and procedures using ICD and CPT codes, ensuring accuracy and adherence to government and insurance regulations.
Required Skills and Qualifications:
  • 3-5 years of experience as a Live Medical Scribe or equivalent administrative role.
  • Administrative writing skills and ability to communicate effectively.
  • Reporting skills and organizational abilities.
  • Record-keeping and Microsoft Office skills.
  • Professionalism, confidentiality, and organization.
  • Typing speed and accuracy.
  • Solid oral and written communication skills.

Benefits and Requirements:
Medical coding certification or successful completion of a certification program.
Bachelor's degree with a pre-health career track preferred.
Strong knowledge of anatomy, physiology, and medical terminology.
Familiarity with ICD-X codes and CPT Procedures.
Skilled in operating various medical record software and hardware, word-processing, and database software programs.
