
Senior Patient Care Specialist

- We are seeking a skilled Nurse to provide high-quality patient care in accordance with established standards and practices.
Key Responsibilities:
- To assess patient health problems and needs, develop and implement effective nursing care plans.
- To administer medications, treatments, and procedures as prescribed by a physician or authorized practitioner.
- To monitor and report changes in patient symptoms or behavior, and take necessary actions to ensure optimal care.
- To collaborate with interdisciplinary teams to plan, implement and evaluate patient care plans.
- To provide health education and counseling to patients and families on disease prevention, management, and treatment options.
- To maintain accurate and confidential patient records, adhering to regulatory requirements and hospital policies.
- To participate in continuous quality improvement activities, identifying areas for enhancement and implementing changes to improve patient outcomes.
Required Skills and Qualifications:
- Bachelor's degree in Nursing (BSN) from an accredited institution.
- Current licensure as a Registered Nurse (RN) in the state of practice.
- Minimum 1 year of experience in nursing, preferably in a healthcare setting.
- Excellent communication, interpersonal, and problem-solving skills.
