Job Summary

The Care Coordination Support Specialist plays a vital role in supporting healthcare providers to deliver high-quality, personalized care to patients. This position involves a blend of operational, clinical, and administrative tasks that ensure seamless coordination between patients, providers, and external agencies. The ideal candidate is detail-oriented, tech-savvy, and passionate about improving healthcare delivery.

Key Responsibilities:

Onboarding Operations Support

- Scheduling Operations
- Retrieve insurance and address details from public databases for new patients
- Monitor visit logs for missed appointments and flag for rescheduling
- Verify scheduled visits are correctly configured in the Electronic Health Record (EHR)
- Update patient contact information in the EHR

Post-Visit Updates

- Attach clinical and nonclinical addendums to respective care plans
- Send Care Plan links via EHR
- Review visit notes for data required in program applications
- Track labs, diagnostic imaging (DI), referrals, and DME orders; flag missing items to providers

External Communications

- Prepare patient notification letters using mail merge and submit for printing
- Maintain and update Excel trackers for patients shared with external agencies
- Send weekly updates to relevant stakeholders

Coordination of Care Support

- Individual Record Review
- Review EHR for new referrals, labs, DI, and DME; update trackers accordingly
- Monitor incoming records and charts
- Identify overdue items and follow up with onshore teams or vendors

Additional Responsibilities

- Execute daily tasks assigned by the supervisor or manager
- Contribute to process improvement initiatives within care coordination workflows

Qualifications

- Experience in healthcare quality operations
- Strong communication skills and a collaborative attitude
- Proficiency in Microsoft Excel
- Prior experience with Electronic Health Records
- Ability to understand and relay medical information and facility protocols
