Role: HCC Medical Coder

Responsibilities:

Review medical records to identify and assign accurate ICD-10-CM diagnosis codes for HCC (Hierarchical Condition Category) risk adjustment.

Ensure coding compliance with CMS, AAPC, and client-specific guidelines.

Abstract relevant clinical information from physician notes, discharge summaries, and other medical documentation.

Validate and update patient diagnosis data to support accurate risk adjustment scoring.

Maintain a high level of coding accuracy and productivity as per quality standards.

Collaborate with auditors, team leads, and physicians to resolve coding queries.

Stay updated on HCC coding guidelines, updates, and regulatory changes.

Protect patient confidentiality in compliance with HIPAA regulations.Role & responsibilities
