**Responsibilities**:
The daily responsibilities of a Resident Medical Officer (RMO) in a hospital to ensure proper medication is given to patients and updated in the medication chart include:

- **Reviewing & Updating Patient Files**: The RMO checks the patient’s medical history, ongoing treatment, and prescribed medications as noted in the patient’s file. They ensure all information is up-to-date and relevant for patient care.
- **Coordinating with Nursing Staff**: RMOs communicate regularly with nursing staff to ensure that the correct medications are administered on time as per the doctor’s instructions. They clarify any doubts or discrepancies the nurses might have.
- **Updating Medication Charts**: After every prescription update or administration of medication, the RMO ensures that the patient's medication chart is accurately updated with the drug name, dosage, frequency, and administration method.
- **Monitoring and Documentation**: RMOs monitor the patient’s response to medications and report any adverse reactions or changes in condition. This information should be documented promptly in the patient's records.
- **Handover**: Ensures that upon patient transfer, discharge, or admission, the written handover is given/taken at the bedside at the beginning and end of the duty shift.
- **Conducting ICU Rounds**: RMOs participate in daily ICU rounds (morning and night) to assess patients, discuss any changes in medication, and update the treatment plan if not done.
- **Patient and Family Education**: When needed, RMOs provide information and explanations to patients and their families about the medications being administered, including potential side effects or reasons for specific medications.
- **Emergency Response**: In case of any medical emergency, RMOs are responsible for adjusting medication (in discussion with the consultant) as needed and ensuring that immediate interventions are carried out properly.
