**Roles & Responsibilities: Field Social Worker (MSW) Note:

- Panjim Goa**

Client Assessment:

- Conduct assessments to understand elderly clients' needs, strengths, and challenges in areas like physical health, mental well-being, social connections, living conditions, and financial situation.

Helpline Coordination:

- Manage helpline calls, both receiving and making, addressing needs and resolving grievances.

Outreach Calls:

- Initiate calls to introduce our organization and services to elderly individuals and their families.

Individualized Care Plans:

- Develop personalized care plans covering social activities, health services, mental health support, and daily living tasks.

Emotional Support:

- Offer counseling to address loneliness, depression, anxiety, and other mental health challenges faced by elderly individuals.

Community Linkage:

- Connect elderly clients with community resources and services, such as senior centers, meal delivery programs, transportation services, and healthcare facilities.

Advocacy:

- Advocate for elderly clients, assisting them in accessing entitlements, benefits, and services within the community.

Crisis Intervention:

- Respond to emergencies and crises, coordinating appropriate responses with relevant authorities.

Home Visits:

- Conduct regular home visits to monitor living conditions and safety, ensuring clients are managing daily tasks adequately.

Family Collaboration:

- Collaborate with family members or caregivers to create a support network for the elderly client.

Social Activities:

- Organize group activities, outings, and social gatherings to reduce social isolation.

Documentation and Reporting:

- Maintain accurate records of client interactions, assessments, interventions, and progress. Prepare reports for supervisors and stakeholders.

Education Workshops:

- Conduct educational workshops for elderly clients and families on topics such as health management, home safety, and available support services.

Continuous Learning:

- Stay updated on trends, research, and best practices in gerontology and social work.

**Requirements**:

- MSW Degree. & BSW Degree
- Field Social Worker experience: 1 year (required).
- Own vehicle.

**Schedule**:

- Day shift.
