Job Description
**Responsibilities and key tasks**
- 1. Wound Care Nurse will be responsible to take care of all wounds in the under mentioned categories:
- Pressure sores
- Non healing wounds
- Infected wounds
- Diabetic wounds
- Leg ulcers (arterial and venous)
- Burn wounds
- Post op wounds -IF infected
- All patients who are at risk of developing pressure ulcers
- Normal and elective wound / clean wound are to be looked after by the primary team under the treating consultant.
- The wound care nurse will assess the wound on visit and simultaneously design the plan of care.
- He/She will be referred the wound from the floor/ICU by the primary nurse.
- Further referral to Reconstructive Surgeon and Primary Consultant will be done by the Wound Care Nurse after his wound assessment if required.
- The Wound Care Nurse trains the Primary Nurse and involve her in the Wound Care Plan of her respective patient.
- He/She will design the Nursing Care Plan for the patient and follow up on daily basis (for IP Patients).
- He/She will have a dual reporting system
- The Wound Care Nurse from all hubs will send daily report to the Wound Care Nurse Manager.
- Wound Care Nurse Manager in turn will submit a weekly report to the Reconstructive Chief and Head Nursing, Quality Training and Education.
- Wound Care Nurse will report to their respective Nursing Heads and Wound Care Nurse Manager.
- This channel to be followed for documented reporting.
- Verbal communication shall take place between Wound Care Nurse and Nursing Head / Reconstructive Team / Nursing Quality,
- Training and Education as and when required.
- The Wound Care Nurse will take a photograph and further discuss the case with the Wound Care Nurse Manager.
- The copy of the wound image will be inserted in patient file for reference.
- All complex wound will be assessed with the help of digital image.
