**Position Summary**:

- The Care Co-ordinator will be responsible for delivering the most remarkable patient experience, in every dimension, every time in alignment with Company’s vision of quality care coordination across the health care continuum.
- The Care Co-ordinator will be central core of the Care Connections Team in collaborating with physicians, mid-level providers, staff, and other health care professionals to provide quality coordinated care across the health care continuum.
- Responsibilities include assisting, developing, and coordinating a wide range of activities of chronic-care management, patient self-management support, as well as acute triage. Position responsibilities are met through providing information pertaining to healthcare needs while promoting health and well-being. The role requires responding on a nurse triage line, assessing problems, and coordinating care as per the set protocol. Some functions may include but are not limited to: responding to general health questions, check on patient compliance and follow up, encourage patient engagement in self-care to reduce the risk of hospitalization.
- The Role of Care Co-ordinator is to utilize nurse triage line for care coordination and dissemination of relevant information, but they will not make medical diagnosis or tell the caller what may be wrong**PRINCIPLE JOB RESPONSIBILITIES**:

- Conduct procedures at home as per needed by the patients
- Capable of working independently with mínimal supervision
- Maintain utmost quality while delivering the service
- Contribute for continual process improvement
- Record additional needs of the patients & document it
- Monitor & supervise a team of bed side assistants
- Plan & execute interventions as & when necessary
- To work as care buddy when needed
- Broad clinical knowledge base of multiple conditions and expected outcomes.
- Ability to demonstrate coordination of appropriate educational materials for patients and their support systems
- Demonstrate appropriate clinical leadership skills as evidenced by previous work-related activities and performances
- Demonstrate the ability to work independently to assess and evaluate understanding of disease process, treatment plans and/or lifestyle changes
- Demonstrate sound knowledge and appropriate decision-making skills for designated patient populations
- Demonstrate appropriate organization skill
- Possesses excellent computer skills with the ability to multitask including navigating and documenting in an EMR while communicating with the patient
- In addition, this person will have strong problem solving and analytical skills and demonstrate communication, organizational, and interpersonal skills
- Tracking patient compliance and follow up
- Clarity in communication and thinking
- Team player and accountability
- Willing for traveling to assess member condition when needed

**Education**:
