**Job Summary**:
**Key Responsibilities**:

- **Patient and Family Support**: Provide emotional support to patients and their families, helping them navigate the challenges associated with brain and spine illnesses.
- **Advocacy & Resource Management**: Advocate for patients’ needs, ensuring they have access to necessary resources and services, including financial assistance, rehabilitation, and community support.
- **Education & Training**: Educate patients and families about medical conditions, treatment plans, and available services. Offer training to staff on how to better support patients in distress or requiring mental health support.
- **Daily Rounds**: Participate in daily rounds with the medical team, providing updates on the psychosocial status of patients and contributing to the overall care plan.
- **Care Coordination & Discharge Planning**: Collaborate with healthcare professionals to develop comprehensive care plans for patients, including discharge planning, follow-up care, and ensuring patients’ ongoing needs are met after discharge.
- **Crisis Interventions & Mental Health Support**: Provide immediate crisis interventions when needed, offering mental health support and connecting patients to appropriate mental health services or counseling.
- **Research & Program Development**: Assist in the development and implementation of new social work programs that enhance patient care, including research on best practices for supporting patients with neurological conditions.
- **Documentation & Reporting**: Maintain accurate and confidential records of patient interactions, care plans, and progress. Prepare reports for senior management and healthcare teams as needed.
- **Collaboration with Healthcare Team**: Work closely with doctors, nurses, psychologists, and other healthcare professionals to ensure comprehensive care for patients.
