Assessment and Evaluation:
Conduct comprehensive assessments to identify the specific needs, strengths, and limitations of elderly clients.

Evaluate physical, cognitive, emotional, and social functioning to determine appropriate interventions.

Care Planning:
Collaborate with healthcare teams, including physicians, nurses, and therapists, to develop individualized care plans.

Create intervention strategies that address the client's physical health, mental well-being, and social integration.

Intervention Implementation:
Administer interventions designed to improve mobility, balance, and physical health through exercises and rehabilitation techniques.

Provide cognitive stimulation activities and memory-enhancing exercises for clients with cognitive impairments.

Offer emotional support, counseling, and therapeutic techniques to address mental health issues such as depression and anxiety.

Facilitate social engagement opportunities and coordinate group activities to combat loneliness and isolation.

Family and Caregiver Support:
Offer guidance and education to family members and caregivers on effective communication and caregiving techniques.

Assist in developing respite care plans to support family caregivers.

Safety and Fall Prevention:
Implement fall prevention strategies and safety measures to reduce the risk of accidents and injuries.

Conduct home safety assessments and recommend modifications as needed.

Documentation and Reporting:
Maintain detailed and accurate records of client assessments, progress, and interventions.

Prepare reports for healthcare providers, families, and organisation.

Continuous Education:
Stay updated on the latest research, techniques, and best practices in elderly care and intervention.

Qualifications:
Master's degree in fields such as Social Work(except CD), **Medical and Psychiatry Preferred**.

Strong communication and interpersonal skills.

Empathy, patience, and a genuine passion for working with the elderly population.

Pay: From ₹10,000.00 per month
