**Job Title: Nurse + Administration (Including Home Health Services and Coordination)**

**Job Summary**:
**Key Responsibilities**:
**Nursing Responsibilities**:
- Provide direct patient care both in the clinic and through home healthcare services, including administering medications, monitoring vital signs, and conducting routine medical checks.
- Assist doctors in clinical procedures, care coordination, and treatment planning for clinic and home-based patients.
- Educate patients and their families on healthcare management, medication adherence, and recovery protocols in both clinical and home settings.
- Maintain comprehensive and accurate patient records, ensuring seamless coordination between clinic-based care and home health services.
- Monitor and manage the stock of medical supplies and ensure the availability of properly equipped home healthcare kits.
**Home Health Responsibilities**:
- Coordinate home health services by assessing patient needs, scheduling visits, and following up on care provided at home.
- Oversee the creation and implementation of individualized care plans for home healthcare patients, ensuring proper communication between caregivers and healthcare professionals.
- Conduct regular home visits to ensure care plans are being followed and to make adjustments as needed based on patient progress or condition changes.
- Act as the liaison between patients, family members, and the medical team, ensuring effective communication regarding care goals and outcomes.
**Coordination Responsibilities**:
- Coordinate daily operations in both the clinic and home health services, ensuring smooth communication between nursing staff, administrative teams, and healthcare providers.
- Develop and manage schedules for clinic staff, home health visits, and ensure proper coverage for all shifts and patient appointments.
- Serve as the main point of contact for coordinating patient referrals, follow-ups, and transfers between the clinic and home healthcare settings.
- Communicate effectively with various teams (nurses, doctors, technicians, and administrators) to ensure that both in-clinic and home health services are aligned with patient needs and clinic policies.
- Oversee the logistics of home healthcare, including transportation, home health equipment delivery, and coordinating with external providers if needed.
**Administrative Responsibilities**:
- Handle patient appointments, schedule management, and follow-up visits in both clinic and home settings.
- Organize and update patient records, ensuring that medical histories and treatment plans are properly documented and communicated across teams.
- Ensure compliance with clinic policies and healthcare regulations, including the proper documentation of home healthcare services for insurance and legal purposes.
- Manage and oversee the procurement of medical supplies and equipment, coordinating with suppliers and ensuring both clinic and home health settings are adequately stocked.
- Support in training staff on both clinical procedures and administrative protocols, ensuring smooth day-to-day operations.
- Assist in patient queries, feedback management, and processing of payments for both clinic visits and home health services.
**Qualifications and Skills**:
- **Nursing Qualification**: Diploma/Degree in Nursing (BSc Nursing, GNM, or equivalent).
- Strong organizational skills with experience in coordinating between multiple teams (nurses, administrators, doctors, and home health services).
- Excellent communication skills, able to manage patient care, scheduling, and coordination across clinic and home health operations.
- Knowledge of healthcare management software and medical record-keeping.
- Ability to multitask and ensure both nursing care and administrative tasks are completed efficiently.
