**Homecare**:

- Conduct clinical assessment and create a care plan for all new clients for homecare and monitor progress.
- Discuss clinical updates with the team regularly.
- Guide the team in achieving the planned clinical outcomes of all patients under the care of KITES Senior Care
- Online consultation for homecare clients on appointment
- Conduct consultation on customers site as and when required.
- Conduct camps as and when company organizes.
- Ensure all documentation is complete as per company protocol.

**Dementia Care Centre**
- Conduct clinical assessment and create a care plan for all new admissions.
- Discuss clinical updates with the team.
- Call the patient family members once a week for updates.
- Guide the team in maintaining and monitoring the clinical outcome of all patients under the care of KITES Senior Care, Dementia Care Centre.
- Regular rounds of all IP patients in the Dementia Care Centre
- Liaison with specialist for clinical update, recommendations
- Preparing Discharge/Death Summaries.
- Ensure all documentation is complete as per company protocol.
- IP Visits (DCC): Mon, Wed & Fri_
- Homecare : Tue & Thru_
- Priyashraya Visit : Sat_

**Job Type**: Part-time
Part-time hours: 20 per week

**Salary**: ₹30,000.00 - ₹40,000.00 per month

Schedule:

- Day shift
- Morning shift

Ability to commute/relocate:

- Bengaluru, Karnataka: Reliably commute or planning to relocate before starting work (required)
