**Position Overview**:
The Social Worker at Prayatna plays a pivotal role in providing holistic support to children and families by coordinating care, conducting assessments, and ensuring a seamless service delivery experience. This position involves working collaboratively with a multidisciplinary team to address the psychosocial needs of clients and their families.

**Key Responsibilities**:
**Client Assessments**:

- Conduct comprehensive intake assessments to identify clients’ developmental challenges, psychosocial needs, and overall well-being.
- Develop client profiles to guide therapy interventions and support services.

**Care Coordination**:

- Assign clients to appropriate therapy departments, including Psychology, Occupational Therapy, Speech and Language Pathology, Physiotherapy, and Special Education.
- Collaborate with multidisciplinary teams to design and implement individualized care plans.
- Monitor progress and modify interventions in consultation with therapists and families.

**Counselling and Advocacy**:

- Provide counselling to parents and caregivers to help them understand therapy processes, set realistic goals, and support their child's development.
- Advocate for disability certification and facilitate access to necessary services and entitlements.

**Discharge and Follow-up Planning**:

- Develop and implement discharge plans, ensuring families have the resources and knowledge required for continued progress.
- Conduct follow-up meetings and review sessions to assess post-therapy outcomes and provide guidance.

**Community Engagement**:

- Organize awareness campaigns and outreach activities to promote early intervention and developmental health.
- Build networks with local medical professionals, schools, and community groups to support clients effectively.

**Documentation and Reporting**:

- Maintain detailed records of client interactions, progress, and assessments in line with organizational protocols.
- Provide regular updates to supervisors and contribute to departmental reports.

**Qualifications and Skills**:

- **Education**: A Master’s degree in Social Work (MSW) from a recognized university.
- **Experience**: Minimum of 1-2 years in a healthcare, rehabilitation, or child development setting.

**Skills**:

- Strong interpersonal and communication skills.
- Proficiency in psychosocial assessments and counselling.
- Ability to work collaboratively in a multidisciplinary team environment.
- Knowledge of developmental challenges and therapeutic interventions.

**Language**: Proficiency in English and Malayalam is essential.

Pay: ₹15,000.00 - ₹18,000.00 per month
