**Role and Responsibilities**:

- To participate more fully in daily tasks, especially when health conditions compromise their abilities.
- Individualized treatment programs based on each patient’s needs, considering physical, social, psychological, and environmental factors.
- Realistic goals with meaningful outcomes and develop strategies to achieve them.
- Collaboration with other professionals and patients’ caregiver to provide holistic care.
- Maintain accurate and up-to-date patient records, including assessment reports, treatment plans, progress notes, and discharge summaries.

**Job Types**: Full-time, Part-time

Pay: ₹30,000.00 - ₹45,000.00 per month
