Description
- Provide direct patient care, including assessing patient conditions, monitoring vital signs, administering medications, and performing treatments and procedures as prescribed.
- Assist patients with activities of daily living (ADLs), such as bathing, grooming, feeding, and ambulation, while promoting independence and maintaining patient dignity.
- Monitor and document patient progress, changes in condition, and responses to treatments, ensuring accurate and timely documentation in electronic medical records (EMR) systems.
- Advocate for patients' rights, preferences, and safety, ensuring informed consent, confidentiality, and respect for cultural and religious beliefs.
- Communicate effectively with patients and their families, providing education and support regarding treatment plans, medications, and self-care techniques.
- Collaborate with physicians, nurses, therapists, and other healthcare team members to develop and implement individualized care plans, ensuring continuity of care and interdisciplinary communication.
- Participate in multidisciplinary rounds, care conferences, and handoff reports to facilitate care coordination and transitions across care settings.
- Adhere to evidence-based practice guidelines and quality standards, participating in quality improvement initiatives, performance improvement projects, and patient safety programs.
- Identify and report adverse events, medication errors, and near misses, contributing to a culture of safety and continuous improvement in patient care delivery.
