The post holder is a Nutrition Coach, who acts within their professional boundaries, working with

a team of health care and management professionals. Key to this role are behaviour change

communication skills and motivational interviewing. You will be the driving force working on the

frontline enabling patients to participate in their own healthcare management, achieve and

sustain their health goals.

As a health coach, you will be

Managing your case load of type 2 diabetes patients

Providing support to patients on daily/ weekly basis through our app or over phone/ video

Supporting patients to make positive, practical changes to their diet and lifestyle

Visiting patients in their homes to conduc initial assessments

Using motivational interviewing to encourage patients to achieve their goal of diabetes

remission

Identifying and working through any potential barriers to attendance and keeping patients

committed to achieve their outcomes throughout the programme

Handling of sensitive data, approaching with great attention to detail

Accurately inputting data into our system to ensure coherent patient pathway

Participating in the Multi-disciplinary team meetings as and when required

Maintaining good inter-personal relationship with other members of the team

Reporting to Dietetic Lead/ Dietitian on a regular basis

A qualified nutritionist or dietitian (freshers are welcome to apply) or have previous

working experience in similar field.

Assertive with good knowledge of basic nutritional concepts with willingness to learn and

undergo training

Collaborative with a can-do attitude and passion for providing remote/ digital support

Innovative and creative providing practical solutions to patients on day-to-day basis in

terms of diet and lifestyle

Confident using technology, specifically digital record keeping

Empathetic towards patients

Strong in oral and written communication

Proficient with one or two regional languages (Kannada/ Telugu/ Tamil/ Hindi)

Flexible with working hours

You will be supported by a team of specialist clinicians throughout and provided with relevant

training to help you succeed in the role
