Job Title: Social Worker - Healthcare Program Coordinator

We are seeking a dedicated and compassionate social worker to join our Delhi-based team.

This position will involve working across multiple healthcare initiatives, focusing on Tuberculosis and Rheumatology. The primary responsibilities will include:

  • Supporting TB and Rheumatology patients with treatment adherence, follow-up, and emotional counseling.
  • Assisting patients in applying for government and non-government aid programs at state and central levels.
  • Helping patients and caregivers with documentation, form submissions, and navigating official processes for availing treatment support.
  • Creating and maintaining a repository of state and national-level patient assistance schemes and health entitlements.
  • Tracking and following up with patients regularly to ensure continuity of care and linkage to appropriate support programs.
  • Coordinating with hospital social work departments, government agencies, and NGOs to address patient needs.
  • Identifying and building a local donor network to support patients financially or in-kind.
  • Maintaining accurate patient records, gathering field-level insights, and contributing to regular reporting and documentation.

Required qualifications include a Master's degree in Social Work, Public Health, or a related field. Relevant experience in healthcare-related social work or public health programs is also essential.

The role involves frequent field travel (~50%) to government hospitals and patient homes in and around Delhi.
