Job Description Job Description : Brief Summary of Job responsibilities in 2-3 lines: Coordination with patients and relatives to ensure a patient centric experience and ensure streamlined process flow in the department. General Job responsibilities : o He/She will be one point contact for patients queries related to the treatment o He/ She will meet patients o assist them with any issues & problems and issues for diet consultation timing etc. should have a checklist based approach. o Act as a liaison between patients and healthcare providers, addressing any concerns or questions regarding treatment plans. o Collaborate with oncologists, nurses, social workers, and other healthcare professionals to integrate counseling services into patients overall treatment plans & ensure holistic patient care. o Communicate effectively with interdisciplinary teams to provide comprehensive support to patients. Patient Counselling o Provide emotional support and counseling to patients diagnosed with cancer and their families during their treatment journey. o To enquire about the patients family status, residential status, psychological status, financial status of the patient o Conduct initial assessments to understand patients psychological and emotional needs. o Develop personalized counselling plans based on individual patient needs and treatment protocols/stages o Coordinate with the Clinical team, for explanations to the patient / relatives regarding the expected side effects of the therapy, guidelines for nutrition, dos and donts during therapy, groups sessions, etc. o Offer guidance on coping strategies, stress management, and communication techniques to patients and their families. o Support the patient psychologically and financially if needed o Facilitate support groups and educational workshops for patients and caregivers. o Maintain accurate and confidential patient records in compliance with hospital and legal regulations. Financial Assistance : o Assess patient's economic status/ send them to PRD with estimate cost of treatment, taking their PRD appointments, communicating with PRD. o Preparing cost estimate certificates under the guidance of medical oncology consultants for outside financial grants. o Coordinating with trusts and individual donor for emergency financial help o Maintain a list trusts/ NGOs to guide patients for financial aids. Cancer Funds : o Donor co-ordination, referring patients to consultants to get assistance from cancer fund Paediatric Patients & Events for Patients o Arranging for gifts during their stay at hospital to make them feel at home o Co-ordinating with MAKE A wish foundation to fulfil their wishes. o Organizing annual fun filled rose day programme National and international patients : o Accommodation facility, tie ups with dharmshala for non-affording patients. o Arranging ambulance services including air ambulance for their return to hometown Palliative Patients: o Counselling patients and family for supportive care o Guiding for arrangements of oxygens, neds, nurses and other support facilities. o Counselling and referring them to shanti Avedna or similar o Referring them to Palcare and Romilla Palliative Care or similar Railway Certificates : o Issue of certificates so that patients can avail the handicap facility provided by Indian Railways. o Fill travel concession forms for to and fro travel of patients. Daily Activities : o Daily Financial, psychological & emotional counselling of patients / caretakers of S1 floors / Daycare, SSS patient. o Coordination with other NGO's for ration and medical aids for deserving patients. o Creating & Facilitating various patient support group & ensuring various activities are conducted. o Daily & weekly activity report to be submitted to Reporting Manager. o Coordinate with departmental staff for any documentation activities. o Coordinating for relevant departmental activities to ensure streamlined functioning of the department. o Any relevant work as instructed by Reporting Manager, Consultants & Head of Department.
