**Patient Support Counsellor (Nutritionist)**

**Job Summary**: In your capacity as a Patient Support Nutritionist, your responsibilities will encompass delivering counselling, education, and support to patients, caregivers, and healthcare professionals.

Your contribution will be pivotal in promoting patient compliance, facilitating therapy sales, and enhancing overall patient welfare. Your role may encompass any of the roles listed below, or a combination thereof:

- Counselling and guidance for patients
- Education and information dissemination
- Collaboration with healthcare professionals
- Advocating for therapy adherence
- Supporting sales efforts

Your duties may involve a combination of these roles, all geared towards ensuring optimal patient care and treatment outcomes.

**Position -** Patient support counsellor

**Location** - Mumbai ,Pune, Ahmedabad

**Experience** -1-2years

**Working hours** - Mon to Sat 9:30 AM to 6 PM

**Key Responsibilities**:
1. **Counselling and Education**:

- Provide counselling related to therapy, including its usage, benefits, and treatment adherence, either at the doctor's clinic or the patient's home.
- Explain and demonstrate the proper usage of the therapy to patients.
- Assist interested patients in purchasing the therapy
- Offer psycho-social support to patients and their families.
- Develop individualized diet plans and schedules to manage patients' conditions.
- Educate patients and families on lifestyle and medical management of chronic diseases.

2. **Device Demonstration**:

- Conduct device demonstrations via video call
- Support the conversion of patient leads via counselling

3. **Onboarding and Support**:

- Assist patients with onboarding as per program scope

4. **Patient Documentation and Coordination**:

- Record patient details and update the information in the Portea portal when necessary.
- Collaborate and coordinate with internal Portea teams to provide timely updates and support.

5. **Patient Care Coordination**:

- Collaborate with all stakeholders to ensure comprehensive patient care.
- Track patient progress, document patient care, and maintain accurate patient records.

6. **Emotional Support**:

- Provide emotional support to family members and other caregivers.

7. **Healthcare System Integration**:

- Work with healthcare systems to ensure continuity of care for patients.

**Qualifications**:

- Bachelor's degree in Nutrition or a related field/ Master’s degree in Nutrition
- Registered Dietitian Nutritionist (RDN) certification preferred.
- Strong interpersonal and communication skills.
- Previous experience in patient counseling and support roles.
- Understanding of medical devices and their usage is a plus.
- Empathy and a passion for patient care.

**Salary**: ₹30,000.00 - ₹35,000.00 per month
