Current Role of Medical Social Worker

As a Medical Social Worker responsible for managing the IPF& Signet health Care fund (SHCF).

Key responsibilities include:

Experience -Fresher or 1 Year Plus Experience

Degree -Bachelor's Degree in Social Work (BSW)

1. Patient Case Assessment & Financial Concession

  • Analyzing patient cases for eligibility under IPF& SHCF-funded medical assistance.
  • Verifying and scrutinizing all necessary documents submitted for financial concession (e.g., income proof, identification, Ration card, Bank Statement, Salary slip/letter, Mediclaim detail, treatment cost).
  • Registering patient cases on the designated online portal.
  • Providing discharge entries on the portal and maintaining post-discharge documentation.

2. Counseling & Family Support

  • Providing financial counseling to patients and their families regarding hospital expenses and available assistance schemes.
  • Guiding them through the application and documentation process.
  • Offering emotional support during financial or treatment-related distress.

3. Coordination with Internal & External Stakeholders

  • Coordinating with the finance team to clear patient dues under the SHCF and IPF fund.
  • Liaising with other hospital departments (e.g., billing, Wards, admissions) to ensure smooth processing of concessional cases.

· Liaise withFinance department for updating concession details

  • Referring eligible cases to external charitable trusts, NGOs, or other funding agencies for additional medical assistance.
  • Maintaining ongoing communication with external partners and stakeholders for patient welfare.

4. In-Hospital Support & Monitoring

  • Conducting regular ward rounds to identify and follow up on patients who may require financial aid.
  • Collecting prescriptions, discharge summaries, and other relevant medical documents from wards.
  • Assisting in shifting eligible patients to earmarked concessional class beds as per hospital policy.
  • Keeping track of pending documents and following up with families or departments for timely submission.

Education -Bachelor's Degree in Social Work (BSW)

5. Documentation & Reporting

  • Maintaining accurate and organized case records, including medical and financial documents.
  • Compiling and submitting monthly reports on SHCF fund and IPF fund utilization, patient support provided, and departmental coordination.
  • Ensuring compliance with SHCF, IPF policy guidelines, and hospital protocols.

Role: Assistant to Social Work Department

Primary Responsibilities:

  • Patient Interaction & Basic Support

· Greet and assist patients and their relatives regarding financial aid and concession procsses.

· Assisting in initial patient and family interaction to collect basic socio-economic details.

· Helping patients fill out application forms for concession or medical assistance.

· Guiding patients and relatives about the document submission process.

  • Documentation & Follow-Up

· Collecting and organizing required documents (ID proof, income certificate, ration card, etc.).

· Following up with patients or their families for missing or pending documents.

· Maintain proper filing of physical and digital records under the supervision of the Medical Social Worker

  1. Ward-Level Support

· Conduct regular ward visits to support social workers in identifying eligible patients.

· Collecting prescriptions, admission notes, and discharge cards from the wards as needed..

· Assisting in shifting patients to earmarked concessional class beds under IPF scheme.

  • Coordination & Communication

· Supporting the social worker in coordinating with other departments like billing, admission, wards, pharmacy, etc.

· Assisting in referring cases to NGOs, trusts, or government schemes when directed.

  • Portal Entry & Data Handling
  • Entering basic patient details into the online portal under supervision.
  • Updating status of document submission and case progress.
  • Assisting in case registration and closure entries under guidance.
  • Administrative & Reporting Support
  • Assisting in preparing daily logs or checklists of patients under follow-up.
  • Helping in compiling data for monthly or weekly departmental reports.
  • Performing other clerical or support tasks as assigned by the department head.

Education -Bachelor's Degree in Social Work (BSW) one year plus experience
