JOB DESCRIPTION FOR MEDICAL SOCIAL WORKER / MEDICAL RECORD
POSITION: - MSW/MRD OFFICER
DEPARTMENT: - MSW / MRD
REPORTING TO: - UNIT HEAD
QUALIFICATION: - ANY GRADUATE WITH DEGREE IN MSW
EXPERIENCE: - MINIMUM 1 - 2 YEAR OF EXPERIENCE
Medical Social Worker :
Job Summary: The Medical Social Worker will provide comprehensive social work services to patients and their families in a hospital setting.
Key Responsibilities:
- Patient Assessment: Conduct thorough assessments of patients' psychosocial needs, including their emotional, social, and financial circumstances.
- Counseling and Support: Provide emotional support and counseling to patients and their families, helping them cope with illness, hospitalization, and the impact on their lives.
- Discharge Planning: Collaborate with medical staff, patients, and families to develop and implement discharge plans that meet the patient's needs and ensure a smooth transition from hospital to home or another care facility.
- Resource Connection: Connect patients and families with community resources such as financial aid, housing, home health care, transportation, and support groups.
- Crisis Intervention: Respond to emergencies, providing crisis intervention and support to patients and families in distress.
- Advocacy: Advocate for patients' rights and needs within the hospital and the broader community, ensuring they receive appropriate care and services.
- Documentation: Maintain accurate and up-to-date records of patient assessments, interventions, and outcomes in compliance with hospital policies and regulatory requirements.
- Team Collaboration: Work collaboratively with interdisciplinary teams, including physicians, nurses, therapists, and other healthcare professionals, to coordinate care and provide holistic support to patients.
- Education: Educate patients and their families about medical procedures, treatment plans, and available support services to help them make informed decisions.
- Program Development: Participate in the development and implementation of social work programs and initiatives to enhance patient care and support services within the hospital.
- Compliance: Ensure all activities and interventions comply with hospital policies, state and federal regulations, and professional standards of social work practice.
Medical Records :
Job Summary: Shall organize and manage the Medical Records Department with appropriate system and provide effective service to the customers.
Key Responsibilities:
- Overall supervision of Medical Records activities
- Responsible for standardization of all the formats used in the hospital for patient care
Coordinates the medical audit activities
Reports the deficiency check information to the management and the clinical heads
- Informing the statistics and information on notifiable diseases to the management and Head
of medical services.
- Approve changes in name of patient, demographic data as per hospital policy. Authorize to
issue photo copies of medical records on patient written request or to relative as per
hospital policy.
- To assist the medical staff and their committees in the hospital in executing the quality
assurance program.
- To collect medical, administrative and other statistics from HIS required by the hospital and
to provide health information for planning and evaluation of healthcare.
- To develop policies and procedures relating to the MRD in accordance with the Health
- Directorate.
- To cooperate with the medical, nursing, head of departments, patients, health agencies,
other hospitals, and legal authorities for the smooth and efficient functioning of the
- hospital in general and of the medical record department in particular.
- To implement information technology (Computer application) for the effective functioning
of the outpatient, inpatient and emergency registration systems to collect clinical and
administrative statistics.
- Birth and Death records notification to BMC
- Notifiable disease notification to BMC
- To supervise the work of MRD staff.
PCPNDT
- Updating monthly CQI and MIS data
To assist the reporting head in different activities, as and when directed from time to time.
Skills:
- Strong assessment and clinical skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of community resources and social services.
- Proficiency in electronic medical records (EMR) systems.
- Cultural competence and sensitivity to diverse populations.
