Theprimary role of the Patient Care Coordinator is to deliver Care bycoordinating between patients on the provider s daily schedule andby proactiveinterventions.
Will act as a conduit for the transmission of information betweenproviders and Customers. Will coordinatewith Medical and Support services for allCustomers. Will facilitate information flowand referral services for customers to address their medical andpsychosocialneeds.
KeyResponsibilities:
Facilitate care by coordinating with the customers for visits andfuture healthcare needs and ensure that proper care is provided toeach customer asrequired.
Handle calls from customers. Resolve the reason for the call orroute it to the respective department as required in a timelymanner.
Provide an effective communication link between customers andmedical staff including relaying messages from providers gatheringinformation from patients for providersetc.
Support in the medication refill process. Ensure that informationgoes when and where it isneeded.
Coordination with clinical and nonclinical support services(Laboratory OT Admission Billing House Keeping Pharmacy) tofacilitateCare.
Ensure that all customers are tracked and data entered into thesystems for followup andreporting.
Coordinate with the medical staff to ensure that case managementservices are provided to customers with complex medical and/orpsychosocialproblems.
Work with the medical staff to develop implement and carry outimprovementprograms.
Act proactively in the quality improvementprogram.
Perform other duties as assigned by relevantauthorities.
Additionaldetails:
Proficiency in verbal communication English and Kannada/Hindi. Anylocal Language proficiency is an addedadvantage. QualificationGraduate/Postgraduate. Experience 0 to 2Years. Demonstrated ability to workeffectively in a team environment.Demonstrated problemsolving skills in a complexenvironment. Demonstrated effectiveinterpersonal relations and customer serviceskills. Good organizational and timemanagement skills. Good working knowledge oflocal social service resources or skills to acquire and use thisknowledge and information expeditiously.
