Position Overview:
We arelooking for a Personal Medical Care Manager (PMCM) who is typicallya
professional who assists individuals with managingtheir medical needs and
healthcarerelated activities.This can include scheduling appointments
coordinatingwith healthcare providers managing medicationsorganizing
medical records and providing support andguidance throughout the healthcare
process.
Roles and Responsibilities:
Conducting virtualappointments with the Clients/patients through an online
platform .
The PMCM will document the details of thevirtual consultation including
assessment findings careplan recommendations and any instructions provided
tothe client in their electronic medical record system.
Develop personalized care plans tailored to each clients medicalneeds goals
and preferences. This may involvecollaborating with other healthcare
professionals suchas specialists therapists or nutritionists. Medication Management: Review medication regimens ensure properdosage
and administration and educate clients abouttheir medications including
potential side effects andinteractions.
Coordination of Care: Serve as a liaisonbetween clients healthcare providers
and family membersto ensure seamless communication and coordination of
care. Schedule appointments
PMCM will collaborate withthe client to develop a personalized care plan. This
mayinclude recommendations for managing symptoms lifestylemodifications
medication adjustments or referrals toother healthcare providers or services as
needed.
Documentation: Maintain accurate and uptodatemedical records including
progress notes treatment plansand care coordination efforts.
Continuing Education:Stay informed about advancements in medical treatments
technologies and healthcare policies to provide the highest levelof care to
clients.
Ethical and LegalCompliance: Adhere to ethical and legal standards inmedical
practice including patient confidentialityinformed consent and compliance with
relevantregulations and guidelines. Skills andPersonal Qualities:

Requirementsand Qualifications:
Bachelors degree (MBBS)
Minimum 5 years of proven experience in clinical.
