**Job Title:** Social Worker for Assisted Living - Activities Coordinator

**Job Overview:**

We are seeking a compassionate and dynamic Social Worker to join our team at [Your Assisted Living Facility Name]. In this role, you will be responsible for designing, implementing, and coordinating engaging activities for our senior residents. Your primary focus will be to enhance the quality of life for our residents through meaningful and enjoyable activities that promote social interaction, mental stimulation, and emotional well-being.

**Key Responsibilities:**

  1. **Activity Planning and Coordination:**

  2. Develop and implement a diverse range of activities tailored to the interests and abilities of our senior residents.

  3. Plan and organize events such as games, crafts, music sessions, educational workshops, and social gatherings.

  4. Coordinate with other staff members to ensure activities align with residents' care plans and preferences.

  5. **Resident Engagement:**

  6. Build strong relationships with residents to understand their needs, preferences, and interests.

  7. Facilitate group and individual activities, providing encouragement and support to maximize participation.

  8. Address any concerns or feedback from residents regarding activities and adjust plans as needed.

  9. **Assessment and Documentation:**

  10. Conduct assessments to identify residents' interests and cognitive abilities to tailor activities accordingly.

  11. Maintain accurate records of residents' participation, progress, and feedback.

  12. Prepare reports on the effectiveness of activities and suggest improvements based on observations and resident input.

  13. **Collaboration and Communication:**

  14. Work closely with other members of the care team to ensure activities complement residents' overall care plans.

  15. Communicate effectively with families and caregivers regarding activity schedules and resident engagement.

  16. Participate in team meetings to provide insights and updates on resident well-being and activity outcomes.

  17. **Compliance and Safety:**

  18. Ensure all activities comply with facility policies, safety regulations, and industry standards.

  19. Monitor and assess activity environments to ensure they are safe and accessible for all residents.

  20. Address any safety concerns or incidents promptly and in accordance with facility protocols.

**Qualifications:**

  • Bachelor's degree in Social Work, Gerontology, Psychology, or a related field (Master's degree preferred).

  • Current state licensure as a Social Worker

  • Experience working with seniors, particularly in an assisted living or long-term care setting.

  • Strong interpersonal skills with the ability to build rapport and engage with diverse populations.

  • Excellent organizational and communication skills.

  • Ability to work independently as well as part of a team.

  • Creativity and flexibility in designing and implementing activities.

  • Proficiency in Microsoft Office Suite and other relevant software.
