We are seeking a detail-oriented and accurate Medical Coder with experience in Evaluation and Management (E&M) coding for both Inpatient (IP) and Outpatient (OP) settings. The ideal candidate will have a solid understanding of medical terminology, anatomy, ICD-10-CM, CPT, and HCPCS Level II coding systems. This position plays a critical role in ensuring compliance and optimizing reimbursement.

Key Responsibilities

  • Review and analyze E&M documentation for IP & OP records to assign accurate diagnosis and procedure codes.
  • Ensure coding compliance with industry standards, client-specific protocols, and regulatory guidelines.
  • Utilize ICD-10-CM, CPT, and HCPCS Level II classification systems for accurate code assignment.
  • Maintain minimum productivity and accuracy benchmarks as defined by the organization.
  • Work closely with auditors, QA, and training teams to enhance coding quality.
  • Escalate documentation discrepancies or unclear cases for physician clarification.
  • Maintain up-to-date knowledge of coding and billing regulations.
  • Support team members in resolving coding-related issues.

Required Skills & Qualifications

  • 1 to 3 years of hands-on experience in E&M coding (IP & OP).
  • Strong knowledge of ICD-10-CM, CPT, and HCPCS Level II.
  • Familiarity with Medicare, Medicaid, and commercial insurance guidelines.
  • Certification preferred: CPC, CCS, COC, or equivalent (AAPC/AHIMA).
  • Working knowledge of medical terminology, anatomy, and healthcare documentation.
  • Strong communication and analytical skills.
  • Proficiency in using EMR/EHR and coding tools.
